FirstPath is a care support program from FirstSolutions. Designed for people with chronic or ongoing health issues, FirstPath uses research-based support methods to improve health outcomes, and help lower overall medical costs. Members receive comprehensive health guidance by specialized RN Care Navigators between clinic visits to ensure the members maintain optimal health.

Whether the issue is chronic headaches, low back pain, depression, diabetes, asthma, a cardiac condition or other ongoing health issues, FirstPath makes it easier to monitor and manage health conditions.

Every step of the way

FirstPath members work with a specialized RN Care Navigator to determine health goals, maintain focus and motivation to reach those goals, accurately monitor health status, and provide timely and important information along the way. This isn’t a pre-packaged program that we simply sell and forget. This is ongoing support and interaction between the member and their personal health support person.
